Im Rahmen dieser Studie sollten die pädagogischen Aspekte einer Patientenschulung mit und ohne Schwimmtraining bei adipösen Menschen mit Diabetes Typ–2 - in bezug auf Stoffwechsel - und Kreislaufparameter sowie das Gesundheitsverhalten untersucht werden.
Material und Methode: Die Untersuchungsstudie wurde über 6 Monate mit N=84 adipösen Menschen mit Diabetes Typ–2 durchgeführt von denen n=41 neben der Patientenschulung zusätzlich an einem Schwimmprogramm teilnahmen. Anhand von Körpergewicht (BMI), Blutdruck und Stoffwechselparametern (HbA1c, Cholesterin, Triglyzeride, HDL, Kreatinin und Mikroalbumin) sollte der Erfolg des veränderten Gesundheitsverhaltens nach 6 Monaten beurteilt werden. Außerdem wurden Fragebögen zur Lebensqualität zu Beginn und am Ende der Studie ausgewertet.
Ergebnisse: Gegenüber den Ausgangswerten ergab sich ein signifikanter Abfall der HbA1c-Werte und des Körpergewichts (BMI) (p<0,001). In der Gruppe mit Schwimmtraining ist die Senkung des HbA1c-Wertes deutlicher, jedoch nicht signifikant (p=0,138). Der Nüchternblutzucker und der systolische Blutdruck konnten zwar nicht nachhaltig gesenkt werden, jedoch lagen sie in der Gruppe mit Schwimmtraining signifikant niedriger
(p<0,001). In der Gruppe mit Schwimmtraining nahmen die Werte für Triglyzeride und Mikroalbumin (im Urin) ab, dagegen das HDL leicht zu, jedoch ohne Signifikanzen aufzuzeigen.
Fazit: Das Schulungsprogramm hat insgesamt positive Auswirkungen auf Körpergewicht (BMI), Stoffwechsellage und Blutdruck. Ein zusätzliches, regelmäßig durchgeführtes Schwimmtraining verbessert den Stoffwechsel noch mehr und stabilisiert auch den Erfolg.

Objective: In this study the educational aspects of patient training with obese Typ-2 diabetics should be examined with regard to metabolic and circulatory parameters as well as the health behaviour.
Material and methods: Over a period of six months, the study was performed with 84 obese diabetics, 41 of them additionally participated in a swim program. From body weight (BMI), blood pressure and metabolic parameters (HbA1c, cholesterin, triglycerides, HDL, creatinine and microalbumin), the success of the changed health behaviour should be assessed after six months. In addition, standardized questionnaires regarding quality of life were evaluated at the beginning and the end of the study
Results: Compared with the initial values, a significant decrease in the HbA1c values and the body weight (BMI) (p<0,001) could be noticed. The patient group who participated in the swim program showed HbA1c values that were more clearly, but not significantly decreased (p=0,138). There was no lasting decrease in the fasting value and the systolic blood pressure value, but these values were significantly lower (p< 0,001) in the swim training group. On the other hand, however, the diastolic blood pressure value of the entire group was significantly lower (p=0,028). The swim training group showed decreasing values for urinary triglycerides and microalbumin, on the other hand the HDL was slightly increased, however without any significance.
Conclusion: All in all, the training program has a positive effect on body weight (BMI), the metabolic situation and blood pressure. An additional, regular swim training improves the metabolism even more and results in permanent success.
